Please plan to arrive 15 minutes early for your first visit to Apex Dental Care in order to complete your registration form and medical history. If you have recent dental X-rays, you may wish to bring them with you. If you are being referred to Apex Dental Care from another dentist, please bring your referral form or letter.
In order for us to better serve and communicate with you, it is important that we have current contact, insurance, and medical information in your file. Once a year, we ask that you review the information we have on file for you, and make any necessary changes. Every three years, we ask that you fill out a New Patient, Medical, and Dental History form.
Important Insurance Information
Dr. Gary Shmurak and our team provide our patients with the best care possible, and an important part of this process is informing patients of their dental needs. Many individuals who are paying for dental insurance do not realize that their plans provide coverage up to a certain dollar amount annually. Consequently, some patients are not scheduling for dental treatment they need, deserve, and have insurance to cover. Unfortunately, numerous patients do not know that insurance benefits for each year cannot be carried over to the next year. The bottom line is that what the patients do not use, they lose. Please contact our dental office in Chalfont, Pennsylvania, immediately to discuss your treatment plan, and our staff will be happy to review your dental benefits at that time.
Methods of Payment and Insurance
Payments for services are due at the time of the visit unless other arrangements have been made. We accept most dental insurance plans. Patients with dental insurance must take care of their part not covered by the insurance at the time of treatment. We will be happy to discuss any special needs in the handling of your account. We accept cash, checks, and major credit cards.
Interest free financing is available.
